Olympia
Moments from Providence St. Peter, designed for short-term stabilization.
- 8-bed pilot residence in a quiet neighborhood
- Thoughtful shared kitchen and living room
- Calm, neutral interiors to reduce overstimulation
Hospital-aligned transitional care
BridgePoint partners with hospitals to place medically stable patients into calm, staffed residences that feel like home while honoring the clinical work you have already done.
For hospitals & care management
We understand the pressure to open beds, align with payors, and still do right by each patient and their family. BridgePoint is designed as an extension of your discharge team—responsive, predictable, and medically appropriate.
Residences
Each BridgePoint home is deliberately updated, warm, and comfortable—more like staying with family than entering another facility. Shared spaces are bright and uncluttered; private corners make room for rest, calls, and reflection.
Moments from Providence St. Peter, designed for short-term stabilization.
Coastal air and a slower pace for those who need time to reset.
Our goal is to help patients stabilize, rebuild routines, and move on—to independent housing, long-term programs, or family reunification. Every stay is purposeful, time-limited, and coordinated with your team.
We start with an 8-bed pilot in Olympia and will scale to additional beds as partnerships deepen.

Referral & Intake
Use this form to send a brief intake summary to our centralized inbox. Please avoid including full PHI if your policies require a secure channel; you can instead request a secure upload link and send the full packet there.
Process
Send us the face sheet, discharge summary, current med list, payor details, and anticipated discharge date. We review same-day for complete packets during business hours.
We confirm acceptance criteria, discuss any nuances, and place a time-limited hold on an appropriate bed, aligned with your discharge window.
Our team coordinates transport, arrival, and medication handoff. We update your case management team once the patient is settled.
Contact & referrals
Share a current discharge challenge, or ask about fit for a specific patient. We are happy to start with a small pilot and build from there.
